Show Me Healthy Women Coordinator

Position Summary: The SMHW Coordinator manages the grant program for MHHC by scheduling and gathering resources and/or services offered by the program to our patients. The individual will work with the community partners and various other programs to carry out the duties required by the position.

The SMHW Coordinator reports to the Chief Executive Officer

Responsibilities:

  • Work directly with patients who qualify for SMHW, SMWW, and any other program created to get them accessible resources and care.
  • Complete and follow up on all necessary documentation for the programs.
  • Bill and track claims using MOHSAIC
  • Establish a strong relationship and communication with the Billing clerk(s) within MHHC to resolve billing discrepancies in the program.
  • Work in collaboration with the Regional Program Coordinator with the Department of health and Senior Services.
  • Provide program education to staff members at all the sites we serve patient care.
  • Establish strong relationships with outside entities to collaborate with for SMHW tests, resources, and referrals.
  • Attend any trainings or meetings for programs.
  • Other duties as assigned.

  • Possess initiative, flexibility, and self-motivation.
  • Excellent oral, interpersonal, and written skills, including preparation of meaningful narrative reports.
  • Capable of being a team member as well as a team leader.
  • Excellent problem solving skills.

Education:

  • High School Education or GED required
  • Minimum of 3 Years of experience in a related health care field
